Vaccine Update

If you know of community memCoronavirusbers that have disabilities and are medically vulnerable, please have them contact the Center for Independent Living of Northwest Florida. Our phone is 850-595-5566 and our email is We are compiling a list for those who have disabilities and are medically vulnerable that want the vaccine. If you have disabilities and are over 50 and would like our assistance in getting you scheduled we can also help you to be on our list too.
The CIL is an approved partner with the State of Florida to host vaccines for medically vulnerable people with disabilities in Escambia, Santa Rosa, Okaloosa and Walton Counties. Please email us at or call 850-595-5566 and we will speak with you to gather needed information. We will need your name, phone number and email address. We will also need your permission to share your information with the state and add you to our list. The State will then contact you with the date and time of your vaccine.
Floridians 40 and older can get COVID-19 vaccine starting Monday, March 29th, all adults 18 and up will be eligible to receive the vaccine starting April 5th, the governor said. Please also message your physician’s office to have them complete the Florida Department of Health’s required form that documents you as being an individual who is medically vulnerable so that you can be added to our list.  SOURCE DOCUMENT:…/covid…/EO-21-47-Form.pdf  
Once you are added to our list, the State will contact you with the date and time for your vaccine. If you have a caregiver(s), they can be vaccinated the same day at the same time as you. We need the same information about them and their permission at the time we put you on the list. We will work together with you for access to services. The CIL’s list is specifically for people with disabilities and if they have caregivers.  As stated earlier in the post please email us at or call 850-595-5566 and we will speak with you to gather needed information.
